Registration

      In order for us to register your child for camp, we MUST receive the following:

• Completed application 

• Completed emergency form

• A copy of your child's most recent physical* (dated within 24 months of attending camp)
• A copy of your child's immunization record*
• Full Payment (Checks made out to Get to the Goal Lacrosse)

   *  Even if your child attended camp last summer- We still need a NEW copy of your child's physical/immunization record.  We do not have the office space to archive physicals from previous summers.
